We believe access to care should never be a barrier to healing, stability, or hope. Through state and community partnerships, we offer grant-supported clinical and peer recovery services for individuals and families who meet eligibility requirements.​

​Grant-funded services are designed to provide short-term, comprehensive support, while helping individuals and families move toward long-term, sustainable care.
A Path Toward Long-Term Care
All of our grant-supported programs are intended as short-term solutions, typically providing services for up to 90 days, depending on the program and individual circumstances.

During this time:

Participants complete required program paperwork

Our team assists with insurance applications and enrollment, when appropriate

Clinical, recovery, and support services are coordinated across care teams

Barriers to care, like transportation or access to services, are actively addressed​

At the end of the grant period, each case is reviewed by our clinical and recovery support team to determine next steps and ensure continued connection to care and community resources.
Our goal is simple: stabilize, support, and transition - so no one is left without options.
